7TH GRADE IMMUNIZATIONS
REQUIRED FOR 7TH GRADE
All students entering the seventh grade are required by the Florida Department of Health to have their adult Td booster shot. This may be obtained by the Broward County Health Department or your physician. You must have a completed 680 Immunization Shot Record illustrating the physician's signature for permanent immunizations.
The completed 680 Immunization Form must received by Indian Ridge before Monday, August 21, 2017.
You may drop off the shot record at the school or mail it to Indian Ridge Middle School, 1355 South Nob Hill Road, 33324. During the summer when school is closed, there will be a drop off box located on the front door.
Please make your best effort to get this done before August 21, 2017. Failure to produce the updated shot record will result in your child not getting their schedule. Students may not attend school without this immunization. Thank you in advance for your cooperation in this important matter.

STUDENT DROP-OFF & PICK-UP
Important Safety Message
For the safety of our students, we implore you to only drop off and pick up your child in the designated drop off and pick up zone.
Please do not park in the faculty parking lots to drop off or pick up your child. It is very dangerous and we fear that a child is going to be struck by a vehicle. Thank for your cooperation in ensuring that all our students are safe.
Thank you!
